Dermal Fillers for Under Eyes: A Complete Patient Guide

You know the feeling. You've tried concealer, slept more, changed your skincare, and still catch the same tired-looking shadow under your eyes every morning. That's usually the point where people start wondering whether dermal fillers for under eyes could help, and whether they're fixing the right problem or just chasing a trend.

The honest answer is that under-eye filler can be a very good treatment, but only when the issue is volume loss rather than pigment, loose skin, or puffiness. The tear trough is one of the most delicate areas in facial aesthetics, which is why the right product, the right depth, and the right injector matter so much more here than they do in many other places on the face.

Why Under-Eye Filler Is on So Many Wish Lists

A common first scenario sounds simple. Someone looks in the mirror, notices the same hollowing under the eyes, and realizes makeup isn't hiding it anymore. The face may look rested everywhere else, but the lower eyelids keep casting a shadow that makes the whole expression look more tired than it feels.

That's why people start researching under-eye dermal fillers after skincare stops helping. Creams can improve the surface, but they can't replace lost structure under the skin. A hollow tear trough is not mainly a skin-care problem, it's an anatomical one.

What filler can and can't do

Under-eye filler is designed to restore volume, soften the shadow created by a depression, and help the lid-cheek junction look smoother. It does not erase true pigment, and it won't fix prominent fat pads that bulge forward. In other words, it can make the area look less sunken, but it can't turn every type of under-eye concern into the same result.

Practical rule: if the problem changes when light hits your face differently, you may be looking at shadowing from hollowing rather than dark color in the skin itself.

The reason this treatment gets so much attention is that the under-eye area is often one of the first places to show facial volume change. A person can feel well rested, healthy, and energetic, yet still look tired because the contour beneath the eye is less supported. That mismatch is exactly what filler is meant to address.

The word “hollows” matters here. People often say “dark circles,” but the actual issue may be a depression that catches light and creates contrast. Once you understand that distinction, the decision becomes less about chasing a specific product and more about matching the treatment to the cause.

The Tear Trough Explained Through Simple Anatomy

An infographic explaining the anatomy of tear troughs, showing the orbital rim, fat pads, and thin skin.

Think of the lower eyelid as a shallow bowl sitting above the cheek. The rim of that bowl is the orbital rim, the bony border of the eye socket. Just beneath it lies the tear trough, the groove where the lower lid meets the upper cheek.

Why depth matters more than product volume

The skin in this region is thin, so even a small placement error can show. If filler sits too superficially, it can look bluish or uneven. If it's placed too low or too much is used, it can create puffiness instead of a smoother contour.

The structure underneath also matters. The area contains delicate tissue layers and fat pads that change with age. When support is lost, the transition from eyelid to cheek becomes more abrupt, and that abruptness is what many people see as “tired eyes.”

The goal is not to erase every line. The goal is to soften the transition so light moves across the area more naturally.

A skilled injector looks at landmarks first, product second. That means identifying the orbital rim, assessing the lower eyelid, and deciding whether the concern is a true hollow, a skin issue, or a bulge that filler could worsen. This is why the tear trough is a precision zone, not a place for broad volumizing.

That precision also explains why placement depth is essential. The same product can look elegant in one plane and obvious in another. In the under-eye area, anatomy drives outcome far more than brand names do.

Filler Types and Brands Used Under the Eyes

The under-eye area is usually treated with hyaluronic acid fillers because they're soft, spreadable, and reversible. The key is choosing a formulation that behaves gently in thin tissue. Higher-viscosity products and permanent fillers are generally a poor fit here because the lower eyelid magnifies irregularity.

The biggest product milestone in this category came on June 5, 2023, when the FDA approved Restylane Eyelight for improving infraorbital hollowing in adults over 21, making it the first U.S. filler specifically labeled for this indication with NASHA technology. That mattered because tear trough treatment had long relied on off-label use in a region where precise placement is hard and the skin is thin. The FDA summary states the device was statistically superior to no treatment for that indication, and independent coverage identifies Eyelight as the first filler explicitly approved for under-eye hollowing. See the FDA-reviewed summary in the Restylane Eyelight approval document for the indication details: FDA-reviewed Restylane Eyelight summary.

How the main options differ in practice

Soft fillers are preferred because they're less likely to look heavy under the eye. Some products are chosen for direct tear trough work, while others are sometimes used to support the cheek first so the under-eye area needs less direct correction. That's why “what's best” depends on anatomy, not just the label.

FillerFDA Status for Under-EyeViscosityTypical Duration
Restylane EyelightSpecifically indicated for infraorbital hollowing in adults over 21Low-viscosity hyaluronic acidVaries by product and metabolism
Belotero BalanceUsed in under-eye treatment discussions, including infraorbital hollow studiesSoft hyaluronic acidVaries by product and metabolism
VolbellaOften favored for a softer swelling profile in thin tissueLow-viscosity hyaluronic acidVaries by product and metabolism
VollureMore often discussed as a mid-face support option than a direct tear trough choiceMid-viscosity hyaluronic acidVaries by product and metabolism

For a deeper look at how injectors choose among filler families, see Athena's guide on which dermal filler is right for your cosmetic needs.

The practical takeaway is simple. Under-eye filler is not about picking the strongest product. It's about choosing the softest formulation that can blend into the tissue without announcing itself.

What the Procedure and Recovery Actually Look Like

A four-step infographic showing the professional process of under-eye dermal filler treatments from planning to final results.

A thoughtful consultation usually starts with facial assessment and planning. In many practices, VECTRA H2 3D imaging helps map the face so the injector can study how the lower eyelid relates to the cheek before any needle or cannula is used. That kind of planning is especially useful in a region where a tiny change can make a big visual difference.

What happens in the room

The treatment itself is usually conservative. An injector may mark safe entry points, clean the skin, and place filler in tiny amounts, often with a cannula or needle depending on anatomy and technique preference. A commonly cited starting point is 0.2 to 0.5 mL per side, then reassess rather than trying to finish everything in one pass. The safest results often come from under-correcting first.

A good under-eye result often looks slightly unfinished on day one. That's intentional.

Early swelling or bruising usually lasts 3 to 7 days, while final settling may take 2 to 3 weeks. That's why the area shouldn't be judged immediately after treatment. The tissue needs time to calm down before anyone can fairly decide whether more product is needed.

A short recovery period doesn't mean the treatment is trivial. It means the region is sensitive. Patients who are prone to under-eye puffiness, allergies, or fluid retention may need even more conservative planning.

For a consultation framework that helps patients ask better questions before treatment, Athena's checklist for questions to ask our Palm Beach plastic surgeon during a consultation is a useful starting point.

Comparing Filler to Blepharoplasty, Fat Transfer, and Lasers

Under-eye filler is one tool, not the universal answer. The right choice depends on what is driving the problem. If the concern is a hollow tear trough with decent skin quality, filler often makes sense. If the issue is prominent fat pads or loose skin, surgery may fit better.

A simple way to triage the concern

Filler is best for volume loss and shadowing. It's minimally invasive, has relatively little downtime, and can be adjusted in stages. That makes it useful for patients who want soft correction without surgery.

Blepharoplasty becomes more relevant when the lower lids are dominated by bulging fat pads or lax skin. In those cases, taking the surgical route may address the anatomy more directly than trying to camouflage it with filler. Athena's comparison of eyelid surgery and tear trough fillers is helpful for patients sorting out that decision.

Fat transfer can be appealing for someone seeking longer-lasting volume restoration, but it's still a surgical procedure and doesn't behave exactly like filler. It depends on how much fat survives after transfer, so results can be less predictable than patients expect from a simple “fill” concept.

Lasers and microneedling don't replace volume. They target texture, pigmentation, and skin quality. That makes them a complement, not a substitute, when the under-eye issue is mostly contour loss.

The cleanest decision-making question is this: Is the eye tired because the area is hollow, puffy, loose, or discolored? If it's hollow, filler may be the right place to start. If it's something else, another procedure may be more honest and more effective.

Risks, Reversibility, and How to Minimize Complications

The under-eye region deserves respect because of what's nearby. The infraorbital foramen, the orbital rim, and surrounding vessels make this a high-stakes area for injection. That's why the best injectors treat it as a precision procedure, not a routine filler stop.

Clinical guidance specifically recommends low-viscosity hyaluronic acid, placed supraperiosteally just below the orbital rim and kept about 1 cm below the rim to reduce visible lumpiness, bluish discoloration, and the chance of accentuating contour irregularities. The same guidance advises caution near the infraorbital foramen and discourages high-viscosity HA or nonbiodegradable agents in this region. See the anatomical review on tear trough correction for those placement principles: tear trough anatomy and placement guidance.

What patients should know before saying yes

Bruising and swelling are the common short-term issues. Overfilling can create persistent puffiness, and superficial placement can cause a bluish cast known as the Tyndall effect. Those are the kinds of outcomes patients usually fear, and they're exactly why conservative dosing matters.

Hyaluronic acid has one major advantage here. It's reversible with hyaluronidase, which makes it the most forgiving filler category for this region. That doesn't mean reversal is routine, but it does mean the treatment has an off-ramp if correction goes too far.

Choose an injector who can describe vessel risk, reversal, and aftercare before they ever pick up the syringe.

A strong safety conversation should include board certification, anatomy-specific training, and a clear plan for complications. It should also include patience, because staged correction lowers the chance of regret. The safest result usually comes from a conservative first pass, then a careful review after the tissue settles.

Cost, Candidacy, and the Consultation Conversation

Under-eye filler pricing varies because the final plan varies. The product chosen, the injector's experience, the number of syringes, and the patient's anatomy all affect the cost. It's also common for patients to need more than a single syringe when both sides need balanced correction.

Who tends to be a good candidate

A strong candidate usually has stable health, realistic expectations, and enough skin quality to let the filler blend smoothly. The area should be free of active infection or irritated skin. The person should understand that filler treats volume, not pigment or texture.

A consultation should feel like a planning session, not a transaction. That's where VECTRA imaging, exam findings, and before-and-after examples help both patient and surgeon decide whether filler, surgery, or a different approach is the better match. Athena also offers financing pathways, including Cherry, CareCredit, and Alphaeon Credit, which can help patients plan treatment more comfortably.

If your main concern is discoloration rather than hollowing, ask that question directly before you choose filler.

The goal is to avoid treating the wrong problem with the right product. That's where patient regret usually starts. A clear consultation reduces that risk by separating contour issues from skin issues before any injection is scheduled.

How Athena Plastic Surgery Approaches Under-Eye Filler

Dr. Avron H. Lipschitz and the Athena team approach under-eye treatment as part of a broader facial planning process. That matters because the lower eyelid rarely exists in isolation. It sits between the cheek, the lid, and the rest of the face, so small changes in support can change the whole expression.

Athena Plastic Surgery works from Stuart and West Palm Beach, and the practice uses VECTRA H2 3D simulated consultations and before-and-after galleries to help patients visualize likely results before committing. That planning style is useful for patients weighing filler against blepharoplasty, laser resurfacing, or skin-tightening options. The result is a more grounded conversation about whether the concern is best handled with injectables or a different path.

If you're starting to explore whether under-eye filler makes sense for you, schedule a consultation, review the photo gallery, and ask how your anatomy changes the plan. A careful evaluation can save you from chasing the wrong fix and help you choose the option that matches what your under-eyes need.

From my very first consultation to every follow-up appointment, my experience with Dr. L (Avron) and his patient coordinator, Terri, has been nothing short of exceptional. Tatiana, his nursing assistant, is also truly amazing. Terri was kind, patient, responsive, and made me feel supported every step of the way. She was the primary factor that promulgated my consideration of Dr. L. Plastic surgery can be overwhelming, but Terri always took the time to answer my questions, ease my concerns, and made me feel like I was her top priority. She is an incredible asset to the practice. This was my fourth plastic surgery procedure, and all of my previous surgeries were performed by well-known surgeons in Miami. I can honestly say that none of them compare to Dr. L. He is just the best in every way and words can’t adequately describe my satiation with his artistry and his medical expertise. Dr. L is, without question, the best plastic surgeon I have ever had. His skills are extraordinary, but what truly sets him apart is who he is as a person. He is humble, compassionate, honest, and genuinely cares about his patients. He never made me feel rushed, took the time to explain everything thoroughly, and his attention to detail is remarkable. Not only is he an incredibly talented surgeon, but he also has a bedside manner that is rare to find. He made me feel safe, confident, and completely cared for throughout the entire process. My results have exceeded my expectations, and even during recovery, I knew I had made the right choice. If you’re considering plastic surgery, I cannot recommend Dr. L and his team highly enough. After four procedures with different surgeons, I can confidently say that I wouldn’t trust anyone else. Dr. L is simply the best, and Terri and Tatiani makes the entire experience even more exceptional. Thank you all for your kindness, professionalism, and for changing my life. I am beyond grateful.

I had a few facial procedures done approximately 3 months ago and am very satisfied. I wanted a natural, refreshed look which is exactly what I received. Terri, the surgery coordinator, was kind and an extremely warm person. No amount of questions were too many. She helped make the process seamless. She always got back to me in a timely manner. Dr. L is a warm, caring surgeon. From start to finish and after surgery, he was always available, even providing his cellphone for any questions or concerns. He is a perfectionist which I truly appreciate. I stayed overnight in the surgery suite with a nurse which was most convenient as I was there for next morning appointment to have drains removed, etc. A nice touch was a delicious smoothie early evening and breakfast the next morning. After the surgery, I had lymphatic massages for a few weeks with Meredith, which really helped the healing process. She too, was a sweetheart. I would highly recommend this practice which I believe was a 10 out of 10!

I’m not one to always leave reviews but I felt compelled in this particular instance. As someone who works in the healthcare field, I came in particular, detail-oriented, and with a very long list of questions. Dr. Lipschitz never made me feel like a burden for it. He took his time at every visit, answered everything thoroughly, and he and Terri stayed responsive and available from consult all the way through recovery. His knowledge and skill are evident — and I genuinely believe his expertise has made my recovery smoother than I expected. So far I am very happy with my results. Dr. Schwartz was wonderful as well. The entire staff — Olivia, Meredith, and everyone in between — played a big part in making this a positive experience. Every interaction felt warm, professional, and reassuring. The whole experience exceeded my expectations and I'm grateful to this entire team. I wouldn't hesitate to recommend them.

I am in the process of working through some significant skin issues and am so grateful to be under the care of Dr. Lipschitz. I have been very impressed with his professionalism and that of his front office and clinical staff as well. In addition, everyone I have come into contact with there is so caring and friendly. I could not be more pleased with the services of Dr. Lipschitz and Athena Plastic Surgery.

I feel very blessed to have found Dr. Lipschitz after researching doctors that do reconstructive surgery after having a stoma and coloscopy. His team is very supportive and professional. He has an extensive knowledge of the human body and is always available to answer any questions or concerns you may have. I highly recommend him for any type of plastic surgery or reconstructive procedures. In a world where doctors are so busy he proves to be accessible, accommodating and supportive of his patients.

Athena Plastic Surgery and Dr. Avron Lipschitz were recommended by several of my personal doctors. I am so happy I took professional advice for whom to use for my face lift and arm surgery. I had a face lift on Jan 20, 2026, and my arms done on Feb26,2026. Both surgeries were done perfectly. I had no complications and the results are amazing. My son says I look 30 years younger. The amazing thing is those who did not know me before surgery can not tell I had a face lift. They compliment me on how youthful I look, or my makeup, but no one believes I am 70, or can tell I had a facelift. The sign of a perfect face lift is others can not tell you had one. My mouth does not stretch clear across my face or my skin is not pulled or puckered. But my frown lines that made me look like I had a hinged puppet mouth are completely gone. My neck looks like it did when I was on my 20s. Dr. Lipschitz and his entire staff are so caring and provide the most wonderful services and bedside manner. I am so glad I decided to follow my doctors recommendations and went with the very best. If you would like to talk to a person who had surgery. Have Terri give me your number and name and you can talk to a real patient.

had a great experience at Athena Plastic Surgery. The doctor was very professional, honest, and took the time to explain everything clearly. I really appreciated that they showed me the product before the treatment, which made me feel confident and safe. The staff was kind, and the whole process felt clean and well organized. I’m very happy with my visit and would definitely recommend this place to anyone looking for quality and transparency.

I’m not even sure where to begin. Dr Lipshitz did a deep planing + facelift on me. To say he is an artist and perfectionist is just the beginning. He is compassionate, calming, and reassuring from consultation through where I am in recovery (which is 6 days out).. He and his entire team treat you like family,. I googled and reviewed more surgeons that I can say and my decision to go with Athena Plastic Surgery was the right choice. I would also like to give a special shout out to Terri, the patient liaison. She is more than my go to girl, she has become a friend. She explains and communicates every minutia detail. She and the Dr both gave me their private numbers and said to text or call any time 24/7. Please if you are looking for someone who is leading edge on all procedures at least talk to them, they will not disappoint.

I’ve had 3 different surgeries recently, and the level of care, and also what Avron did is just amazing. He is very clear on what he is doing, and aftercare is AMAZING. I recently also started some Botox treatments and Meredith is fantastic. All the staff is great and friendly. I HIGHLY recommend Athena for your cosmetic needs!!!!!

Gotta say Dr. Lipschtiz is the man… your in good hands… he works miracles and I’m happy with my nose… I had a dog bite scar that split my nose and for many years had to walk around like Frankenstein… it wasn’t a cool scar… After surgery I felt like I finally got rid of the monster… it was a big relief… I am eternally grateful and will sing him praises and give this man his flowers for life… so that’s my 2 cents if your on the fence… its well worth it!!!!

I am a 76-year-old woman who never had any surgeries before but wanted a full-face lift in my final years on this planet. I interviewed several plastic surgeons and the only one I felt 110% safe and confident with was Dr Lipschitz. He explained ALL to me from the beginning to the end of the procedure. He was there 24/7 after the surgery and beyond. If I needed anything or had any issues he was there. I felt safe and confident that I would be just fine and I was. He helped me calm down and heal in a caring manner. He never complained or felt I was a bother to him. I am so pleased with my results, and I promise you that you will not be disappointed with this dr or his caring staff. This dr is a true masterpiece with his hands and skills to make me look young again at my age. If you don't use this dr you will regret it.

In being a first timer on January 7, 2026 with Athena Plastic Surgery. My orthopedic Dr as well as my surgeon. He recommended Dr. Lipschitz for help with a non healing incision that was touched by a therapist to soon after surgery. Dr. Lipschitz, is a caring, passionate, and knowledgeable in his field. Today, February 11, the incision is much better than my actual first visit. I am so grateful for his expertise in getting me back on a healing track. Athena has a great staff Brandi, Rachel, Olivia, front desk personnel (don’t want to butcher the spelling of her name - lol) were the main ones I dealt with. I had no bas experiences during my time. I didn’t get a couple of returned calls; however it was not that serious to penalize them. I definitely recommend Athena for beauty enhancements with Dr. Lipschitz. Thank you so much for your service. 100000/10

I have gone to Meredith for Botox about 3 times. She does an excellent job of pinpointing exactly where I need it and….she is fun!! Sometimes it can be a little intimidating getting self improvement services done but Meredith was so kind and natural. If you are thinking about Botox I highly recommend going to Meredith. And make sure to book a morning appt. She will get your day started with a laugh and a positive outlook! Thank you, Meredith.😀

I cannot say enough good things about Athena Plastic Surgery and Doctor Lipschitz, as well as his team. If you go here for anything, you will be in good hands. I can’t believe we have one of the very best plastic surgeons in the whole world, right here in Stuart, Florida.

I am so pleased with my experience with Dr. Lipschitz and his team! I’m a teacher and new to the area. One of my child’s parents referred him to me. It wasn’t necessarily an easy case. I had been in a good deal of pain and felt very self conscious. I was so pleased, he listened to me, discussed my options and I was on the books! I felt like I was in good hands and I was! I had gone to other surgeons who ran through a countlessly given speech…I was just another patient. I am very happy with Dr. Lipschitz’s work and I wouldn’t hesitate to recommend him.

I went to Dr. Lipschitz years ago for fillers and I really appreciated that he educated me on both short-and-long-term results. This is why I chose him to do some more maintenance work recently, and once again, he acted with integrity. It was a big decision for me and he and his staff answered my questions candidly and treated me with professionalism and care. The best part is the significant improvement his artistry has made on my appearance, confidence and life overall. Dr. Lipschitz is one of the top plastic surgeons in the country, you can read up on his degrees, honors and extensive training (indcluding a fellowship at Johns Hopkins). I couldn't imagine going anywhere else and I couldn't be happier with my results.
