The patient is a 23 year old male who sustained nasal trauma with resulting significant breathing difficulties and no previous surgery. On evaluation he had a wide bony base, significant septal deviation right internal valve collapse with + Cottle maneuver, turbinate hypertrophy, dorsal hump, disruption of the DAL on the right. The patient underwent septoplasty, turbinate resection, rhinoplasty to include, osteotomies, narrowing the bony base dorsal hump reduction and right spreader graft. This is a beautiful 57-year-old female business owner presenting for facial rejuvenation and revision rhinoplasty. The patient underwent primary rhinoplasty more than 20 years before presentation and has breathing difficulties and complains of nose deformity. On analysis of the nose she has a supratip fullness residual dorsal hump and poor definition of the tip and wide bony base. The patient underwent revision rhinoplasty with revision of the supratip deformity, dorsal hump reduction osteotomies and reshaping of the tip with a combination of suture technique and cephalic trip as well as septoplasty and turbinate reduction. She underwent deep plane facelift at the time of rhinoplasty surgery. The patient is seen here 6 weeks post operatively and is still swollen but overall contour improved.

This is a 19-year-old female who underwent septo/rhinoplasty. She presents with breathing difficulties and would like to improve the appearance of the nose. On examination she has a prominent dorsal hump and bulbous tip. An open septorhinoplasty was performed including osteotomies, dorsal hump reduction spreader grafts and tip refining sutures. Images are before and 4 weeks post op. We can expect further improvement after swelling comes down over the next several months.

He has significant breathing difficulties due to septal deviation and turbinate hypertrophy as well ad dorsal hump and deviation. This is a 21-year-old male before and 6 weeks after open septo/rhinoplasty. The patient complained of a bump on his nose (dorsal hump) as well as difficulty breathing. Internal nasal exam demonstrated deviated nasal septum, turbinate hypertrophy causing obstruction of the nasal airway. On facial analysis the patient has overall excellent facial aesthetics with a strong jawline and chin and good overall facial proportions. He has a wide nasal bony base and dorsal hump but good tip definition and nostril proportions. This rhinoplasty consisted of septoplasty, turbinate reduction, dorsal hump resection and osteotomies to narrow the bony base. Rhinoplasties are individualised and customized for each patient and no 2 rhinoplasties are alike. We take into consideration overall facial aesthetics, gender, patient desires and functional aspects of the nose among many other variables. Ultimately the goal is for a functional and structural improvement matching the nose to the patient's overall facial features.

This is a 74-year-old patient Coordinator in a plastic surgery office. The patient presents for facial rejuvenation and rhinoplasty. Images are before and 3 months post op. The patient complained of some functional problems including difficulty breathing through the right side of her nose and contour problems including a slight dorsal hump, nasal deviation or crooked nose, disruption of the right dorsal aesthetic line, a bulbous bifid tip and retracted ala and hanging collumella with excessive nostril show. An open septo rhinoplasty with placement of right spreader graft to correct right nasal dorsum dorsal aesthetic line , reduction of the dorsal hump, cephalic trim, tip refining sutures inter and intra domal sutures, caudal septal resection and placement of ala rim grafts.

The patient sustained nasal trauma years before surgery and had difficulty breathing. On analysis she has significant nasal deviation, Dorsal hump, wide bony base and shortened tip with poor tip definition. Internal exam shows significant septal deviation and turbine hypertrophy. Dr Lipschitz performed septoplasty and rhinoplasty straightening the nose with a combination of osteotomies and spreader grafts. The dorsal hump was taken down giving an overall improved aesthetic. The nose matches the aesthetic face.

This is a 33-year-old mother of 2 presenting with difficulty breathing through her nose and requesting to improve the contour of the nose. On examination there is a dorsal hump, wide bony base, over projected, bifid tip. Here we work to bring the nose in proportion to the rest of the face with an open septo-rhinoplasty. The images are before and 3 months post op and contour can be expected to improve further over the next year. The patient underwent dorsal hump reduction, osteotomies, cephalic trim, and tip suturing techniques to refine the tip. The septum is also addressed to improve functional aspect of the nasal airway and improve breathing. The overall result demonstrates a more balanced appearance to the facial shape. In rhinoplasty form and function are improved to create a more harmonious facial aesthetic.
